Medical Release:

I fully realize that injury or illness to my child may result from or during participation in the youth camp. In case of injury or illness, I give permission for my child to be given medical treatment as deemed appropriate. I further give permission for the information provided on this form to be shared with appropriate medical personnel. I further give permission for and grant authority to the camp representatives to sign on my behalf the Notice of Privacy Practice that patients are required to receive in accordance with federal law. I understand and acknowledge that I will be responsible for any medical bills incurred by my child.

Waiver and Release of Liability:

In consideration of being allowed to participate in any way in the Greg Coplin Golf School Summer Golf Camp, the undersigned acknowledges, appreciates, and agrees that:

1. Assumption of Risk: I understand that participation in golf activities carries inherent risks, including but not limited to physical injury, which may be minor or severe. I voluntarily assume all such risks associated with my participation.
2. Release and Waiver: While my child is participating in the Greg Coplin Golf School Summer Golf Camp, I acknowledge and assume all the foregoing risks on his/her behalf and likewise accept personal responsibility for any injury or damages that may occur. I, for myself, my heirs, personal representatives, or assigns, hereby release, waive, discharge, and covenant not to sue Greg Coplin Golf School LLC, its administrators, agents, sponsors, other participants, advertisers, and owners/lessors of premises used to conduct the activities (collectively "Releasees") from liability from any and all claims, including negligence, resulting in personal injury, accidents, or illnesses (including death), and property loss arising from, but not limited to, participation in the Greg Coplin Golf School Summer Golf Camp.
3. Indemnification: I agree to indemnify and hold harmless the Releasees from any and all claims, actions, suits, procedures, costs, expenses, damages, and liabilities, including attorney’s fees, brought as a result of my involvement in the Greg Coplin Golf School Summer Golf Camp and to reimburse them for any such expenses incurred.
4. Medical Treatment: I hereby consent to receive medical treatment deemed necessary if I am injured or require medical attention during my participation in the Greg Coplin Golf School Summer Golf Camp. I agree to be responsible for any costs associated with such medical treatment.
5. Personal Property: I acknowledge that I am responsible for the care and safety of my personal property and that Greg Coplin Golf School is not responsible for any loss or damage to personal property.
6. Photographic Release: I grant and convey to Greg Coplin Golf School all rights, title, and interest in any and all photographs, images, video, or audio recordings of me or my likeness or voice made by Greg Coplin Golf School in connection with my participation in the Summer Golf Camp.
7. Compliance with Rules: I agree to comply with all stated and customary terms and conditions for participation. If I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately.

 I have read the above waiver and release, and understand that I have given up substantial rights by registering.
